2024-2025 Fundraising Form

1. No Fundraising activities under the auspices of Halton Hills Minor Hockey may start until:

      A. Approval received by HHMH
      B. Any HHMH Logo use must receive explicit permission from the VP Administration, HHMH.

2. The dollar amount of the fundraiser must be included in your team budget and attached to this request.

3. Policies set out by HHMH will be adhered to during all fundraising as per the policies and guidelines found on the HHMH website.

4.  Logo use of any kind must be approved by the VP, Administration. Logo details section must be filled out below.

5.  Bottle drives will be approved by Director, Fundraising.  Bottle drive details must be filled out below.

(raffle, 50/50, car-wash, bottle drive, community BBQ, golf tournament, buggy boys at local grocery store, etc.)
