This form is to request approval for 3rd party instructors for your team that have not been previously approved by HHMH.
Note:
All third-party instructors must be pre-approved by HHMH. Please ensure all credentials are accurate and relevant to the proposed training.
Only rostered third-party instructors will be approved to work with HHMH teams. Submission of this form does not guarantee approval. Approvals will be sent via email.
