25-26 Goalie Equipment Request Form

Goalie Equipment Request

This form should be filled out by the Coach of the H/L Team or parent of child requesting equipment

Any equipment borrowed, must be returned as indicated by the Director of Equipment at the end of the season in reasonable shape considering type of use.

If there is excessive damage or loss, due to neglect or willful damage, the individual may be charged for replacement.

Please fill out one form per required item.

If you are a parent and require equipment and are having trouble acquiring it, OR have a general question regarding the sign out of equipment, please contact Rob Hornblow (Director of Equipment).
