2024-2025 Permission to Skate Form

If you are planning to try out for AAA or OJHL team, you need to be Registered with Hockey Canada For the 2024-2025 season and obtain an approved Permission to Skate Form from Halton Hills Minor Hockey.

By clicking the box below you understand that you will receive a Permission to Skate Form only if you are registered with Hockey Canada & HHMH, for the 2024-25 season.

DO NOT PAY ANY FEES TO HHMH UNLESS, AFTER AAA TRY-OUTS ARE FINISHED, YOU ARE RELEASED FROM THE AAA TEAM YOU ARE TRYING OUT FOR AND WILL BE PLAYING FOR HHMH.

If you pay by credit card and subsequently selected by the AAA team you are subject to a $25 admin fee to process your refund from HHMH.
